Practice Charter

Please ask at reception for a copy of our practice charter.

How Can You Help Us?
• Be on time for your appointment - the doctor may not see you if you are more than 10 minutes late.
• Tell us if you need to cancel your appointment - there may be another patient who needs it!
• Call for a home visit or urgent appointment before 10.30am.
• Ring for test results after 2.30pm.

Confidentiality
We ask you for personal information so that you can receive care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

Comments And Suggestions
Should you have any comments or suggestions you would like to make on the facilities available at Neptune Health Park, please contact the practice and we will be happy to pass on your comments to the relevant Neptune partner.

Complaints
The Black Country Family Practice aims to give a friendly and professional service to our patients. However, if you have any concerns about any aspect of our service, please let us know.

Speak to whomever you feel most comfortable - your GP, the practice nurse, the patient service manager or our reception staff will be happy to help.

In the majority of cases, concerns can be resolved quite easily. However, if you feel we have not dealt with the issues you have raised as you would wish, you can write to the complaints manager at Sandwell Primary Care Trust.

The PCT also operates a Patient Advice and Liaison Service (PALS), which can often help resolve any problems before they become formal complaints.
